Dear Applicant,


Please complete all the following steps in the application process:


  1. Go to the Forms page on our website.
  2. Please complete the General Application Form.
  3. After you complete and submit this form, you will be prompted to pay a non-refundable $100 application fee.

Note: We cannot process your application without this fee.

  1. Ensure your son completes the online Talmid Self-Evaluation Form.
  2. Ensure your son’s principal completes the online Principal Evaluation Form.
  3. Email your child’s grade 7 & 8 report cards to
  4. The Emergency Medical Form is a PDF, rather than an online form. Please print it, complete it, scan it and email it to
  5. Email a recent picture of your son to

NOTE: Your application will not be processed until you have completed ALL of the above steps.

  1. We will email parents to schedule a formal (online) Farher test and interview for all applications that are considered.

An acceptance email will be sent when an applicant is accepted. The following fees are due upon acceptance:

  1. $275 final registration fee: we will provide the instructions for paying this fee in your acceptance email
  2. $175 security damage deposit: payable by check to The Thornhill Mesivta.

In the memo area of the cheque, please write: “Security deposit for [your child’s full name]”

  1. $350 annual mikvah fee: payable by check to LYO. Please mail to the same address in the previous step.

In the memo area of the cheque, please write “Mikvah fee for [your child’s full name]”

Alternatively, you can pay the mikvah fee from the Chabad Southern Ontario Donation page. In the Other field at the bottom of this page, enter: “Mikvah fee for  [your child’s full name]”


Note: Talmidim must ensure they understand the yeshiva’s guidelines and dress code which are outlined in the Student Handbook.
